This aircraft was involved in an accident on Oct 31, 1945. On a flight from Rio de Janeiro, Brazil, to Montevideo, Uruguay, and Buenos Aires, Argentina the propeller of the no. 3 engine separated. Debris struck the no. 2 engine and a propeller blade sliced through the fuselage. Two passengers were killed and a small fire erupted. An emergency landing was made in a lagoon in Uruguay. The aircraft was repaired and returned to service.
